Open Water Rx serves patients across the United States. Available nationwide in all 50 states. Some states have additional requirements before a prescription can be issued.
Mississippi patients: live video visit required
Mississippi state law requires a synchronous (live video) visit with your licensed provider before any prescription can be issued. After completing your intake, you will be required to schedule and complete a brief video visit with your provider. This is a state-specific requirement and is not optional. Prescriptions are issued only after this visit is completed and your provider determines that treatment is clinically appropriate for your individual health profile. Additional time may be required before your first prescription.
Other states may also require a synchronous initial visit depending on current telehealth regulations. You will be notified clearly if this applies to your state after your intake is submitted.

Open Water Rx offers three distinct clinician-guided program categories, each designed for specific health goals and patient profiles. Selecting the right program starts with understanding your primary objective: meaningful weight management, energy and longevity support, or discreet men's health solutions. Every program includes independent licensed medical provider review, transparent all-inclusive pricing, and ongoing support. No insurance is required for any program.
Weight Loss Programs are our most popular option and are designed for adults with a BMI of 27 or higher who are seeking medically guided weight management. Our standard GLP-1 programs feature compounded semaglutide at $169 per month all-inclusive or compounded tirzepatide at $219 per month — both flat monthly pricing with no membership fees or separate provider fees. Both medications have demonstrated significant efficacy in clinical trials with FDA-approved formulations. Compounded versions have not been evaluated in the same trials. These programs include comprehensive provider review, medication shipped discreetly if prescribed, and ongoing async support.
Longevity and Energy Programs focus on cellular health, metabolic support, and healthy aging through NAD+ nasal spray and sermorelin injections and orals. These programs are designed for adults who want to proactively support their energy levels, recovery capacity, and long-term cellular health. Longevity programs start at $112 per month and are prescribed only when a licensed provider determines clinical appropriateness based on your individual health profile.
Men's Health Programs provide discreet, clinician-reviewed options for men's sexual health. Multiple formulation options are available, and all programs maintain complete privacy from intake through delivery. Sildenafil and tadalafil $39 for a 1-month supply; Quattro Sublingual $69. Patients must be 21 or older and complete a full health evaluation reviewed by an independent licensed provider. Prescriptions are issued only when clinically appropriate.

Receive your treatment plan
If approved, your provider issues a prescription and your medication ships from a licensed pharmacy partner directly to your door.
Frequently Asked Questions About Getting Started
Can I switch programs after enrolling?
Yes. If your provider determines that a different program would be more clinically appropriate, you can switch at any time. Your provider will guide this decision.
Do I need a referral from my primary care doctor?
No referral is required. Open Water Rx operates on a direct-to-patient model. However, we encourage you to inform your primary care provider about any new medications.
How long does the intake take?
The online health evaluation takes approximately 5 minutes. Provider review typically occurs within 24 to 48 hours, though some cases may take longer.
Is there a commitment or contract?
No. All Open Water Rx programs are month-to-month with no long-term contracts. You can cancel through your patient portal at any time, and no advance notice is required.
What if I don't qualify?
Nothing is charged unless your medication is approved. If a licensed provider determines treatment is not appropriate, you are not charged.
